ARRIVAL: Let yourself in with the door code 4569* - don't forget the star! Please use the restroom if needed and have a seat to wait.
HEALTH & SAFETY: If you've been exposed to COVID-19 within 10 days, please wear a well-fitting mask to your appointment. Please do the same if anyone in your home is sick. If you're feeling sick, please cancel your appointment with as much notice as possible.
PARKING: Use the parking entrance to the left of my office building and park in an open space to the left. Technically you'll park behind the building next door.
NEW PATIENTS: Please complete the health history and intake paperwork prior to your appointment time. Be sure you've eaten something before your appointment and dress comfortably.
CANCELLATIONS: I require notice of cancellation no less than 24 hours in advance of your appointment time. Arriving more than 15 minutes late may be considered missing an appointment. Late cancellations and missed appointments may be charged at the full rate to the credit card on file. By confirming your appointment you agree to this cancellation policy.
